Describe what occurred in the order it occurred. Include sensory details, duration, presence of others, and any physical aftereffects. Describe what you observed; leave interpretation aside.
Species present, when each animal began to respond relative to your first awareness, indoors or outdoors, the type of response, any animals that stayed unaffected, and how the response ended.
The interval between your first awareness of the object and your first attempt to record it, and any thought or impulse that arose in that interval.
Number of other observers present, if any. Names are optional.
Note any physical evidence: ground traces, biological effects, recovered material, photographs, audio and video.
An email address if you wish to be contacted for follow-up. Optional, and kept private.
Submissions are reviewed and logged. Anonymized entries may be published in the civilian documentation network.
