This form is for citizens documenting anomalous encounters, UAP observations, NHI contact events, rights violations, biological exposure incidents, and related phenomena. Submissions are reviewed and logged in the civilian registry. Documentation does not obligate further action. The record is maintained regardless of whether follow-up occurs.
Document before you discuss. Memory degrades faster after conversation than after sleep.
Separate observation from interpretation. Record what you saw, heard, and felt. Record conclusions separately.
Include time, location, and duration even if approximate.
If biological exposure occurred, seek medical assessment before submitting. See Appendix B, Biological Hazard Protocol.
If you are uncertain about classification or documentation standards, read Appendix B of the HYBRIDS GUIDE before submitting. Proper documentation strengthens the integrity of the civilian registry and ensures your account is logged accurately.
Section 1
Event Details
Provide the basic coordinates of your incident — when it happened, where, and for how long. Approximate values are acceptable. Precision aids cross-referencing with other reports in the registry.

Approximate Time
[Time field]
Location
City, region, or coordinates if known [Text field]
Duration of Event
[Short text field]
Section 2
Incident Classification
Select the category that most accurately describes the nature of your encounter. If your incident spans multiple categories, choose the primary type and describe additional dimensions in the account field. Classification helps route your report to appropriate reviewers in the registry.
UAP / Craft Observation
Visual or instrument-detected sighting of an unidentified aerial or transmedium craft.
Direct NHI Encounter
Face-to-face or physical contact with a non-human intelligence or entity.
Indirect NHI Presence
Sounds, electromagnetic field effects, missing time, or behavioral anomalies without direct visual contact.
Biological Exposure
Physical effects on the body, skin contact, or suspected biological or radiological exposure following an event.
Rights Violation
Non-consensual interaction, coercive contact, or suppression of documentation or testimony.
Memory Interference
Screen memory suspected, non-sequential recall, or gaps inconsistent with normal sleep or consciousness.
Additional Categories
Ongoing / recurring phenomena
Property or equipment effects
Other — describe in account field
Witness Count
[Number field] — Enter total number of witnesses including yourself.
Were Other Witnesses Present?

Section 3
Your Account
This is the primary record of your experience. Write in sequence — what you perceived first, then what followed. Sensory detail matters: sound, smell, temperature changes, physical sensations, and any effects on your body or nearby equipment. Do not interpret here. Interpretation is recorded separately in the next field.
Describe in sequence. What you saw first, then what followed. Include sensory detail — sound, smell, temperature, field effects on your body or equipment. Do not interpret yet.

Section 4
Interpretation
This field is optional. Keeping observation and interpretation separate preserves the evidential integrity of your account. Reviewers assess the raw observation first, then consider interpretive notes. If you have a working hypothesis — a classification, a name, a framework — record it here without embedding it into the factual account above.
If you have a classification hypothesis, note it here. This is separate from the observation record and will be reviewed independently.
[Optional text area — interpretation field]
Section 7
Follow-Up Contact
Providing contact information is entirely optional. If submitted, your email address is used solely for follow-up communication regarding this specific report. It is not stored in any shared database, not used for outreach unrelated to this submission, and not shared with third parties under any circumstances.
If you choose not to provide contact information, your report is still received, logged, and reviewed. Anonymous submissions carry equal weight in the civilian registry.
Preferred Contact for Follow-Up
[Email field — optional]
Privacy Notice
Contact information is not required. If provided, it is used only for follow-up on this submission and is not shared.
Anonymous Filing
You may submit without any identifying information. The record is maintained in full regardless.
Data Retention
All submissions are retained in the civilian registry indefinitely. No submission is deleted upon review completion.
Submit Your Report
Review your entries before submitting. Once received, your report is assigned a registry number and enters the review queue. Not all submissions receive a direct response, but all are logged in the civilian record.
